Unique Disability Identity cards[UDID] are issued to Persons with Disabilities through competent medical authorities notified by respective State Governments/Union Territories. Who Can Apply
As per the Persons with Disabilities Act of 1995, you can get a card if you have one of the following disabilities:

Leprosy Cured | Cured of leprosy but still have loss of feeling or movement in hands, feet, eyes, or eyelids, with deformities or weakness, yet able to do regular activities. |
Locomotor Disability | Difficulty moving limbs, joints, muscles, or bones, often caused by conditions like cerebral palsy.
